Waiver of Liability, Release, Assumption of Risk & Indemnifying Agreement

This is deemed to be an agreement by the parent/guardian and the skater to assume the risks and hazards incidental to these activities.  We understand and appreciate that the risk of injury from ice skating and playing hockey is significant, including the potential for serious injuries including permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce the risk, this risk of serious injury does exist.  By my, or my child's participating, WE KNOWINGLY ASSUME ALL SUCH RISKS, both known and unknown.  We therefore, WAIVE, RELEASE and HOLD HARMLESS Jamie Todd, the Outlaws Hockey Club (OHC), and all persons acting on his behalf including; organizers, coaches, supervisors , South Suburban Parks and Recreation District, its employees, agents, contractors and all volunteers (all of the foregoing being "volunteers") from any and all liability for bodily injury or property damage arising out of or resulting from participation in the OHC program.  We attest that the named skater is physically fit to participate in all OHC activities.  My signature below is to certify that I, as parent/guardian with legal responsibility for this participant, have read, understand, and agree to all that is stated in this waiver.  It also certifies that my child and I have read, understand, and agree to the OHC Rules & Guidelines that are attached to this form.  This release and indemnity is intended to be as broad as permitted by law.
